SERVICES:

As a Licensed Clinical Social Worker, I am committed to pursuing social change and challenging social injustices through anti-oppressive practices and cultural humility. Using a combination of techniques and skills from ecological systems theory, cognitive behavioral therapy, and dialectical behavioral therapy, I offer a trauma-informed and healing-focused approach to assisting those struggling with grief & loss, depression, anxiety, work-life balance, life transitions, parenting stress, communication, and relationship conflict. My practice is LGBTQIA+ affirmative, meaning that I will offer you dignity, respect, and affirmation.

I use a narrative approach to managing an identified problem or issue, which allows you as the client to determine if, how, and when you want to try something new or different in your life. Mindfulness is a part of each therapy session, as mindfulness skills are the most accessible way to identify one’s feelings, needs, and reactions/responses to others. If applicable, I may also offer guidance towards additional client-directed supports such as journaling, meditation, yoga, healing arts, spiritual guidance, community work, and other complementary services.

In addition to individual therapy, I offer clinical supervision to social workers seeking licensure or ongoing professional development. Please inquire for availability.

FEES:

  • Initial Assessment: $200

  • Individual Psychotherapy: $150

I am an in-network provider for Blue Cross Blue Shield PPO and Blue Choice of Illinois. You may also have the option of using other health insurance (see note below).

If you have limited financial resources and/or do not have mental health insurance coverage, I reserve appointments for clients in need of sliding scale fees through Open Path Collective. Please visit their website for additional information on affordable therapy.

At any given moment, you have the power to say: ‘This is not how the story is going to end’
— Christine Mason Miller

A note regarding health insurance:

Currently, I am in-network with Blue Cross Blue Shield of Illinois PPO and Blue Choice. However, if you have a different health insurance provider with mental/behavioral health coverage, you may have the option of paying out-of-pocket for sessions, and submitting the statement to your insurance provider for out-of-network coverage reimbursement.

If you wish to seek reimbursement through your insurance provider, be sure to confirm coverage by logging into your account on the insurance website, or calling customer service and asking about “out-of-network benefits”. Ask about your deductible and any relevant co-pay or co-insurance responsibilities. Confirm any pre-authorization requirements, required documents or referrals, and/or other limits to care.

Good Faith Estimate

Under Section 2799B-6 of the Public Health Service Act, health care providers are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage, both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

  • You have the right to receive a “Good Faith Estimate” explaining how much your care will cost.

  • Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises
